Eligibility Requirements and Application for Individuals Seeking a Scholarship
The Recovery House Scholarship Program is intended to address the needs of individuals stepping down from substance use disorder (SUD) residential treatment services (Levels 3.1-4). To be approved, the applicant seeking assistance through this program must require financial assistance to move into a recovery house as part of his/her plan of recovery from substance abuse.
Eligibility requirements for the scholarship include:
- 1. The applicant is being referred by a credentialed professional from a licensed SU Residential facility (Rehab or Halfway House) or by their SCA Case Manager (if applicable)
- 2. The applicant has been a resident of Cumberland, Dauphin, Lancaster, Lebanon, or Perry County for no less than two (2) consecutive months immediately preceding the date of the scholarship application, excluding time incarcerated, hospitalized, or in a treatment facility
- 3. Applicant is or was enrolled in Medicaid HealthChoices
- 4. The recovery house noted on the application is both licensed by the PA Dept. of Drug and Alcohol Programs (DDAP) and enrolled in the CABHC Scholarship Program
